Job Announcement New York City Elder Services

Heights and Hill Community Council, a 38-year old community-based agency serving the frail elderly in Brownstone Brooklyn is expanding our case management program to serve a much larger area in Brooklyn. We are looking to hire 2 Team Leaders/Case Management Supervisors and 8 Case Managers. All positions will begin April 1.


(2) TEAM LEADERS/CASE MANAGEMENT SUPERVISORS (MSW)

The Team Leader works under the supervision of the Executive Director and alongside the Assistant Director. The team leader supervises staff and volunteers and will represent the agency in community and interagency activities, developing and maintaining strong community ties for the designated geographic area.

The Team Leader is the facilitator who ensures that day-to-day operations are carried out in an efficient manner and who keeps the Executive Director fully informed concerning staff needs, morale, and program effectiveness. As a member of Management Staff, the Team Leader works closely with the Assistant Director and the Executive Director in program planning and development.

8) CASE MANAGERS:

The Case Manager works under the supervision of the Assistant Director or the Team Leader/Supervisor and plays a key role in ensuring that agency programs are carried out effectively and efficiently. The Case Manager will work closely with other Heights and Hill staff concerning program effectiveness and together with the Executive Director, Assistant Director, Team Leaders and other team members, plan and carry out agency activities and programs. The Case Manager will, with the approval of the Team Leader, attend community meetings, attend training sessions, participate in committees and work with professional staff of other agencies and organizations that provide services to the elderly.

Qualifications:

The Case Manager must be a professional individual with knowledge of social work theory, knowledge of entitlements and resources and be capable of making independent decisions and reacting to crisis situations in a professional and appropriate manner. The Case Manager position requires a Bachelor’s degree in Social Work (BSW) or human services field; some experience with the elderly preferred. Prefer bi-lingual and cultural competency in Spanish, Chinese or Haitian Creole.

Qualifications : The Team Leader/Supervisor must be a dynamic, professional individual with a thorough knowledge of social work theory, a working knowledge of community resources and entitlements and benefits, and be capable of making independent decisions and reacting to crisis situations in a professional and appropriate manner. The Team Leader position requires a Master's degree in Social Work plus four years experience in human services, preferably with elderly; prefer at least one year experience

Team Leader Responsibilities
Performs screening and intake of all new potential case management clients
•Assign caseloads and related duties.
•Provides case management services to a small caseload of homebound frail elderly.
•Supervise social work/case management staff.
•Review case records of case management staff for DFTA standards compliance and clinical appropriateness.
•Works with Assistant Director to identify staff needs and develop in-service trainings to meet those needs
•Supervise and coordinate activities of volunteers and student interns.
•Represent the agency at community meetings
•Reach out to community leaders, community organizations and service providers to make them aware of Heights and Hill services, as well as to identify those in need of services and identify gaps in service in the community
•Forge community partnerships to enhance the life of seniors in the community
•Work with the Executive Director and Assistant Director on special projects.
•Compile statistics, prepare reports, keep attendance and other required records.
•Perform other duties as required.

Case Manager Responsibilities

•Provide accurate and appropriate information on services, benefits and entitlements to the clients.
•Conduct in-home client assessments and follow-up visits to homebound seniors in need of services.
•Develop and follow through on care plans.
•Arrange for provision of services, advocate and intervene with other agencies.
•Maintain complete and accurate computerized case records and statistical reports.
•Develop and maintain liaison with other agencies and resources.
•May supervise and coordinate activities of volunteers and student interns.
•Perform other duties as required.
